A tricyclic glycopeptide antibiotic considered a drug of last resort for treating serious, multi-drug resistant Gram-positive infections, most notably MRSA (Methicillin-Resistant Staphylococcus aureus) and Clostridium difficile colitis (oral formulation). It inhibits the final step of bacterial cell wall synthesis. Vancomycin requires therapeutic drug monitoring (TDM) due to its narrow therapeutic index and risk of nephrotoxicity and ototoxicity.
